Fill out the employment application. You can click the button to the right to take you directly to the form or find it under job seekers in the menu.

UPLOAD 4 FORMS FOR DOCUMENTATION
Download each of the following forms (W-9, I-9, Vaccination, and COVID Form). Once filled out, scan onto your computer, then upload each file accordingly. Please press submit after uploading all files. Upload supported file (Max 15MB)

DOWNLOAD EMPLOYEE LETTER & UPLOAD FILES
Download employee letter and upload Driver's License and Resume, followed by whichever license applies to you (X-Ray, Hygienist, Dentist, or CDA/RDA License), Then upload (if any) CPR or Nitrous Oxide Certificates. Please press submit after uploading all files.

DOWNLOAD EMPLOYEE CONTRACT & SIGN ELECTRONICALLY
Download employee contract, read over terms and conditions, and sign using electronic signature. Please press submit, this confirms Dental Connections has captured your signature.
